Section titled “1. The COD Button or Embedded Form Does Not Appear on the Storefront”- Check Theme App Embed Activation: Open Home (
/app) and confirm the Theme App Embed badge is Active. If inactive, click Enable in Theme Editor and save your live theme. - Check Display Surface (

2. Native Shopify Buttons (Add to Cart / Checkout) Are Still Visible Despite Hiding Enabled
Section titled “2. Native Shopify Buttons (Add to Cart / Checkout) Are Still Visible Despite Hiding Enabled”- This is usually the Fail-Safe Native Button Guard working as designed.
- Whenever COD is ineligible for the current product, cart mix, buyer country, or subtotal, the storefront coordinator refuses to hide native Shopify purchase buttons so your store does not lose non-COD sales.
- Once the product/cart satisfies all COD visibility rules, native button hiding applies automatically.
3. Order Submission Returns FORM_CONFIG_REVISION_MISMATCH
Section titled “3. Order Submission Returns FORM_CONFIG_REVISION_MISMATCH”- This occurs when you save a configuration change in Admin while a buyer (or your own test tab) already has an older revision of the form open.
- Refresh the storefront page so the browser loads the latest
cod_form.v2_snapshotrevision and submit again.
4. No Shipping Rates Available / Order Rejected at Shipping Gate
Section titled “4. No Shipping Rates Available / Order Rejected at Shipping Gate”- Open Form Builder → Shipping Rates (
/app/form-builder/shipping-rates). - Confirm at least one shipping rate covers the selected delivery country and province.
- Check the rate’s
minSubtotalMinorandmaxSubtotalMinorconditions—remembering that conditions are evaluated against the discounted merchandise subtotal (after Quantity Offers), not the pre-discount price.
5. Test Orders Are Blocked During Store QA
Section titled “5. Test Orders Are Blocked During Store QA”- Open Fraud Protection (
/app/fraud). - Check if your test submission triggered Order Velocity Limit (default
3orders per24h) or Duplicate Order Detection (default72h) or exceeded Maximum Product Quantity Limit (1..10). - Add your office or test IP address to Trusted IP Addresses in
/app/fraudand click Save.
